How Do You Want Your Journey to End?

The answer to this question is as individual as a fingerprint. The bigger question is how will you ensure that your wishes are known and followed?

Advance care planning is the process that allows you to make your end-of-life choices known. These plans provide direction to healthcare providers when you aren’t able to either make or communicate your wishes.

It’s also important to discuss your wishes with your loved ones, especially the people you designate as your healthcare proxy. Making sure they understand your wishes will help guide any decisions they have to make in the future. Advance care planning can help you and your loved ones avoid the stress of making uninformed decisions in difficult situations.

The Center for Education & Advance Care Planning is pleased to be part of Honoring Choices® Indiana – North Central. The group’s purpose is to help residents of Elkhart, Marshall and St. Joseph counties understand advance care planning and how to initiate conversations about their future health care wishes. Learn how to make sure you’re prepared for your finale.
