Advance Care Planning Process Tools

Are you ready to get started? Even if you aren’t, please take a moment to get familiar with the resources that are available to Indiana residents to help with advance care planning. The links below provide directions and forms.

As part of the process, here are some other questions you may want to consider as you plan:

  • What are the most important things you want those close to you to know about your end-of-life care? Be sure to talk about them and include them in your documentation.
  • Who will speak to your care team (doctors, nurses, etc.) if you’re not able to? Make him/her aware of your wishes and where your advance care documents are located.
