The Center for Education & Advance Care Planning is here to help you help your clients by offering tools, resources and current information on a variety of end-of-life planning topics. We also offer a number of continuing education resources to increase your knowledge and skills in the field as well as generate ideas that will benefit your clients – and your business.

In addition, we offer a number of activities designed to help area experts increase their network of other professionals in the end-of-life arena.

Become an Advance Care Planning Certified Facilitator

Throughout our service area there are certified facilitators who are available to educate and facilitate discussions and guidance to individuals and families who wish to develop an advance care plan, complete advance directives and designate a health care representative.
