Next First Steps ACP Facilitator Certification Class
Day 1: October 29, 2019 – 8:00 a.m. to 4:00 p.m.
Day 2: November 6, 2019 – 9:00 a.m. to 12:30 p.m.

This First Steps ACP Facilitator Certification is designed for professionals such as social workers, nurses, clergy, physicians and others involved in assisting patients and residents in making end-of-life treatment decisions.
Purpose: To improve the effectiveness of conversations with individuals regarding end-of-life treatment decisions and Advance Care Planning through facilitation skills training.
Registration begins: August 1, 2019
Registration ends: August 31, 2019
Cost: The total cost of the program is $200 which includes registration fee, lunch and take-home materials. Make checks payable to Honoring Choices Indiana – North Central.
